Individual Formula Tailoring

The WIC Infant Formula Tailoring Calculator assists in individually tailoring infant formula issuance. This calculator can be used in conjunction with existing State agency developed breastfeeding assessment forms and tailoring formula policy/procedures.

The WIC Tailoring Calculator is designed for WIC Competent Professional Authorities (CPAs) to:

• Identify if a formula tailoring opportunity exists,
• Identify potential causes, tips/solutions for breastfeeding concerns, and referral opportunities,
• Determine nutritional needs and infant feeding patterns, and
• Provide follow-up to assure the mother can meet her breastfeeding goals.

The CPA will work with the Designated Breastfeeding Expert (DBE) to develop an appropriate care plan to help the mother achieve her goals.

Tailor the amount of infant formula based on, but not limited to: 

• Assessment of mom’s milk production, baby’s intake, and supplementation practices.
• Recommendation from the baby’s healthcare provider (HCP).
• Mom’s understanding and agreement of breastfeeding plan and amount of formula issued while the breastfeeding concerns/challenges are addressed.
